The Excel version can be filled in and emailed to us. You will be asked if you want to open or save. If you choose open, Excel will open and display the form. If you have Excel 2007 there will be a Security Warning prompt at the top of your screen. Click on Options and select Enable This Content to allow the spreadsheet to editable. (See link below.) If you have an older version of Excel you will have to enable macros.
